COMMON QUESTIONS

WHAT IS YOUR NEW PATIENT PROCEDURE? ... and ... YES! YOU CAN RENEW WITH US EVEN IF WE WERE NOT YOUR FORMER PROVIDER!

1. Visit the patient pre-appointment form at www.CannabisCardClinic.com to set up a callback from one of our Patient-Advocate Medical Assistants who are Certified Medical Marijuana Consultants.

2. Please answer our call...we'll be calling from (808) 798-3629. We will answer all of your questions and if you so desire, we'll set up an appointment that is convenient for your schedule. Be at our offices at or a few minutes before your appointment time. We have dozens of free reserved parking stalls and two handicapped stalls at the main entrance. If you are infirmed you can request a housecall.

3. THINGS TO BRING: Bring your CURRENT photo-ID: State ID, Driver's License or Passport. Please bring medical records that show your qualifying diagnosis. Some patients bring in their current prescription bottles as well. Your appointment will include seeing your medical marijuana doctor who can assist you if you do not have any pre-existing medical records available. This may require reasonable medical testing for verification and treatment planning.

WHO WILL I SEE AND WHAT ARE THEIR QUALIFICATIONS?

Your first contact will be with the founder of the Honolulu Wellness Center, Paul Klink*, who will assist you in creating your application, collect your pre-existing medical records, educate you with information about the medicine and answer any questions allowable. You will then have a face-to-face bona fide  Doctor + Patient relationship launched at this appointment with either Dr. Michael Pasquale or one of his associate physicians who are board certified licensed physicians in the State of Hawaii who will ask pertinent questions for primary diagnosis verification, if possible verify your qualifying diagnosis, educate, and provide answers to related questions.
